On one hand, his zero tolerance policy and unrelenting drive can be interpreted as inspiring and a significant catalyst toward groundbreaking change in the field of medicine. On the other hand, working for Harvey or being around him is not for the faint of heart. On a personal level, he is extremely ambitious, competitive, and dedicated to his performance, his image, and the legacy he has worked so hard to prolong. He has little interest in social activities, and his personal nearsightedness has deprived him of the opportunity and desire to fall in love or raise a family. “Harvey, they’re ready for you.”

“Says who, you? Is there evidence or did you just conceptualize that in your corrupted mind?” The dark-haired man peered over his desk to meet the eyes of his closest friend and right hand in the hospital, who also happened to be the head of the psychiatric department. Not gaining enough of a reaction from the man for the way he was disrespecting his specialization, Harvey paused, grew a smug smile, then added, “my inner child is still finishing up this paperwork.”

“Ha ha. Very funny.” The bright-eyed doctor stepped into the room further, glancing down at the task that the gloomy gentleman was actually performing: sorting the applicants the hospital had accepted into piles based on how likely he believed they were to fail. When he raised an eyebrow at the sheer volume of applicants he’d placed in each department’s respective ‘first to go home’ category, the seated gentleman rolled his eyes.

“Don’t act like I’m so off base here, Jesus Christ. You focus on your mind miracles while the rest of us are here in reality.”

“Whose reality,” the psychiatrist quipped. “Yours? I’ve been in this hospital long enough to know that it’s not a nice place to be. But I’ll go back to turning wine into water, or whatever.”

Not so long after Maxwell Arthur’s exit from his office, Dr. Harvey exited on his own power and paused in his doorway on the highest level of the hospital, peering over the intricate glass architecture to examine what was occurring in the first floor lobby, which was the only space large enough to host all of the applicants that his overly optimistic and indecisive panel had elected into the program. When he saw the sheer volume of the masses that had accumulated in that space, he exhaled, asking himself why he hadn’t found the time to simply accept and reject the large group of under qualified doctors himself. Taking the elevator down to the second floor, Harvey accepted a microphone from Dr. Isaac, another close acquaintance of him who worked as the head of both pediatrics and neurosurgery. Ysabel was quite married to her job here, the one thing that Dr. Harvey had in common with the few people he trusted enough to take high positions in this establishment.

“Since I don’t need to introduce myself, I’ll keep this short. Look around you, and see everyone who has been accepted into this program. There are over five thousand of you that have been accepted at this point. By the end of the day, less than half of you will remain. By the end of the week, less than a quarter. The very best doctors in the entire world want to come here to study and practice medicine, so you have to show me why you belong here with them. I don’t tolerate mistakes, and I don’t tolerate excuses, so if you’re looking for this to be a pleasant experience for you, I suggest you find your nearest exit now. For those of you who want to prolong the misery for yourselves, the competition begins right now. Your first task is to save my life.”

Before Max, Ysabel, or any of the other doctors could make their way across the upper-level hallway to take the mystery syringe out of his hand, the tall figure injected something into his body, only instructing the department heads to pay close attention to their students to discern who to fire before dropping unconscious on the ground, completely limp and lifeless. As the department heads watched, roughly half of the students rushed forward in their regular clothing to try to fight their way into Dr. Harvey’s immediate vicinity, discarding their backpacks, jackets, and purses in the central lobby and waiting room. The other half either meekly waited for the doctor to stand back up, assuming he hadn’t actually done anything to put himself at risk, or simply because they did not want to confront what had turned into a competitive, angry mob of doctors looking to treat their professional idol. Others seemed terrified, annoyed, or in despair at the prospect of what the head of the hospital had described the competition as, and had taken his advice in leaving the hospital and the competition before anything had even really began. Among this large group, Dr. Arthur observed several familiar faces, some which he’d seen in the applications he’d combed through during his time on the panel, and others from individual interviews which he’d conducted, along with other panel members, for the past year or more in anticipation of this moment. Still another, a younger blonde women, he recognized from another place: he’d just seen her photo in the pile of applicants Dr. Harvey had labeled ‘first to go home,’ though he couldn’t directly remember which department her specialization was in. Of course, even the slightest knowledge and preparation placed him high above the position that the applicants were currently in, especially when Dr. Harvey had sneakily scheduled appointments for each department with important figures that kept them incapable of stepping in and helping the applicants solve their case for them. What was even more overwhelming for the applicants was that Dr. Harvey had never explicitly told them to be ready to work that first day, and some had even come in still carrying their suitcases. To them, it was supposed to be a relaxing, short orientation day. To Dr. Mason Harvey, it was an opportunity to teach them that working at his establishment meant being ready for high-pressure situations at any time, regardless of perception or interpretation.

Madeleine had watched Dr. Harvey from the back of the room, her arms crossed as he delivered his speech. There was an intensity in his voice, a coldness she’d come to expect from the man who had become almost legendary in medical circles. His challenge wasn’t a surprise—everything about this program was designed to break the weak. But as he continued, there was a flicker in her gut, something that told her this was more than a simple test of competence. It was something deeper, something calculated.

When Dr. Harvey pulled out the syringe, Madeleine’s heart skipped. Her eyes narrowed, a mix of curiosity and unease settling over her. What is he doing? The doctors around her tensed, sensing the shift. Then, without hesitation, he injected himself. A collective gasp rippled through the crowd, and for a brief moment, silence hung in the air before the room erupted into chaos.

Madeleine stood amidst the crowd, her breath catching slightly as Dr. Harvey collapsed onto the floor. The room had shifted in an instant from an assembly of hopeful doctors to chaos, as half of the candidates surged forward, their eyes wild with desperation. Her ocean-blue eyes flickered between the unconscious figure of Dr. Harvey and the mob, calculating her next move.

Her heart raced, but she forced herself to remain calm. The hysteria around her was palpable—doctors clambering over one another in their scramble to reach Harvey. She watched them, some shoving others aside, their professionalism forgotten. It was clear that most of them weren’t thinking—just reacting. That wasn’t going to work.

Think, she reminded herself. Rushing blindly into the fray would only result in adding more chaos. She inhaled slowly, pushing her nerves aside, and her mind began to race through possibilities. If this was a test, as it so clearly seemed to be, then there was something more to it than just a medical challenge. Dr. Harvey wouldn’t be so reckless.

With a quick glance around, she moved with purpose toward the periphery, where a group of candidates still stood frozen. They were hesitant, unsure whether to join the frenzy or to remain passive observers. Madeleine’s fingers itched to dive in herself, to find out what exactly had been injected into him. But she knew better than to get caught up in the chaos.

Taking charge, she turned to the group beside her, her voice cutting through the noise with authority. “We need a plan. If we all rush in, nothing will get done. We need to work as a team.”

Some stared at her blankly, fear etched into their features, but a few nodded, their eyes brightening as they recognized the sense in her words. She pulled off her scrub jacket and tossed it aside. “Cardiac or neuro first. Who’s got access to the toxins in the hospital supply?” Her mind raced with every bit of knowledge she’d gathered over the years.

“Are we really going to let them take the lead?” someone asked, motioning to the doctors already swarming Harvey. Madeleine shook her head.

“They’re panicking. Let them try, but we’ll be the ones who actually save him,” she said, her lips curving slightly, a touch of her usual charm showing. Her focus, though, was razor-sharp. She needed to figure out what Dr. Harvey had injected himself with. Her mind flickered to her toxicology training—if it was an intentional overdose, they’d need to act fast. “Get me any syringes they’ve used or discarded. And someone keep an eye on his vitals.”

Her eyes flicked over to where Harvey lay, limp and lifeless, and for a brief moment, a pang of worry tightened her chest. She couldn’t afford to be distracted by fear. There was too much at stake.

The first step, as always, was observation.
